Job Description:
A company is looking for an LVN Case Manager in Disease
Management to facilitate a team approach for quality care
delivery.
Key Responsibilities
Conduct clinical evaluations and assessments to identify member
needs and gaps in care
Develop and implement case management plans in collaboration with
members and healthcare professionals
Maintain a member caseload for regular outreach and management,
ensuring compliance with service standards
Required Qualifications
Graduation from an accredited Licensed Vocational Nurse program
Current unrestricted LVN license in California
2+ years of recent clinical experience as an LVN/LPN
1+ years of experience in care management, disease management, or
discharge planning
Access to a dedicated workspace with high-speed internet
connection
