  - 1 Completes an initial assessment of patient and family to determine home care needs.
  
  - 2 Regularly reevaluates patient nursing needs
  
  - 3 Initiates the plan of care and makes necessary revisions as patient status and needs change
  
  - 4 Uses health assessment data to determine nursing diagnosis
  
  - 5 Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
  
  - 6 Includes the patient and the family in the planning process
  
  - 7 Initiates appropriate preventive and rehabilitative nursing procedures.
  
  -- 8 Administers medications and treatments as prescribed by the physician

  Qualifications:
  • Graduate of an accredited school of nursing. One to Two years of recent acute care experience in an institutional setting
  • Current RN license in the State of Colorado
  • CPR Certified
  • Bachelor's Degree, with one year of home health care experience preferred.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills, etc.
  • Reliable transportation and license to drive.

    -Rate determined by years licensed up to 10 years.
