Job Description:
A company is looking for a Care Progression Manager - BSN to
oversee patient care progression and resource management.
Key Responsibilities
Oversee a population of in-patients and progress care through
discharge using evidence-based criteria
Identify patient clinical conditions and interventions that can be
completed in the ambulatory space
Manage care and resources to reduce unnecessary utilization and
facilitate outpatient testing and follow-up
Required Qualifications
Bachelor's Degree in nursing
5 years of experience in an emergency department, including adult
and pediatric care
5 years of experience in in-patient acute care hospital
Licensed Registered Nurse in Pennsylvania upon hire
