Job Description:
A company is looking for a Referral Nurse Navigator (RNN) to
serve as the primary liaison between clients and specialists,
managing referrals and consultations.
Key Responsibilities
Submit and manage eConsults on behalf of primary care providers
Route referrals and communicate responses from specialists to
primary care providers
Monitor overdue eConsults and assist in clinic workflow
improvement
Required Qualifications
Bachelor's degree preferred
Current, unrestricted LPN, LVN, or RN license required
2+ years' experience in the nursing field
Familiarity with eConsults and referral management
Proficient with major EHR platforms and Microsoft/Google Suite
