Any exceptions are at the discretion of the Program Director.Completed PVCHC APRN Primary Care Fellowship ApplicationMust be graduates of an accredited family nurse program (by orientation date -- with less than 750 hours of supervised prescriptive practice, as defined by Colorado DORA) or transitioning FNPs who demonstrate an intent to practice as a primary care providersMust have at least 2 years of RN work experienceCertification in Family Nurse Practitioner SpecialtyActive & Unrestricted Colorado RN & APRN license (eligible and complete by orientation date)Three (3) letters of recommendation - One (1) from a clinical preceptor, One (1) from an academic professor from a NP program, and One (1) from a professional reference.
